Proof of Social Security Number

Confirm your Social Security number (SSN) by sending us a copy of one of these documents. You also have options if you don’t have an SSN or your SSN application is pending.

Your eligibility notice or account message will tell you who in your household needs to submit proof of your Social Security number and how to send it.

You have a document with your SSN.
Social Security card Tax form Benefit or income statement from Social Security that shows the Social Security number
Your SSN is pending or unavailable.
Tax form Benefit or income statement from Social Security that shows the Social Security number
You have an established religious objection.
Letter stating that the applicant refuses to obtain an SSN for established religious objections (also called an SSN Waiver)
